Overview
Probably one of the documents that are commonly requested and required in transactions like job application, school enrollment, travel documents and other government-issued ID registration or application is the birth certificate. A birth certificate is an official document which can be used to verify your identity and nationality and most of all your age for legal purposes. So how and where can you request your birth certificate here in Philippines? First, let’s define what a PSA Birth Certificate is.
What is PSA Birth Certificate?
PSA stands for Philippine Statistics Authority which is a Philippine government agency responsible for the regulation and governing the production of statistics, civil registration and identification system of the country. It is also responsible for the national censuses and surveys, sectoral and community-base statistics, consolidation of selected administrative recording system and compilation of national accounts. Among the civil registry documents which are regulated by the PSA include the following:
- Birth Certificate
- Marriage Certificate
- Death Certificate
- CENOMAR (Certificate of No Marriage)
A Birth Certificate is an official document which contains your birth details including your full name, gender, place of birth, date and time and your biological parents. By the name itself, this certificate is generated upon your birth wherein the doctor or any authorize medical staff files this birth report to the Local Civil Registry Office (LCRO). After filing and registration of your live birth, you will then be issued with an original copy of the Birth Certificate. Afterwards, the LCRO endorses this report to PSA for archiving and regulation. Thus when a Birth Certificate was called as PSA Birth Certificate, this means that the certificate was generated by PSA agency and therefore certified as true copy or representation of your original record. In the absence or loss of your original birth certificate, you can always request a certified true copy of it from PSA which is printed in SECPA (Security Paper).
Now that you know what a PSA Birth Certificate is, let’s find out the things you need to prepare prior to requesting your PSA Birth Certificate.
Requirements in Getting PSA Birth Certificate (Via Online Appointment System)
This process of acquiring your PSA Birth Certificate requires scheduling an appointment to your preferred PSA branch before you can get one. Here’s what you need in scheduling your appointment:
- Mobile phone or computer with internet
- Preferred schedule in mind
- Printer (for printing your Appointment Slip)
On the day of your appointment, here’s what you need to bring:
- One valid ID (Driver’s License, Postal ID, Employee ID, NBI Clearance, Passport, Senior Citizen ID, SSS ID, GSIS ID, PRC ID, IBP ID)
- Authorization Letter from owner (If your claiming the certificate for someone else)
- Accomplished Health Declaration form (only applicable for this period due to current COVID-19 situation)
- Processing Fee of ₱150.00
- Printed CRS Appointment Slip
Now let’s schedule your appointment. Here’s your step-by-step procedure on how to schedule your appointment.
How To Schedule an Appointment Online To Get PSA Birth Certificate
Step 1
First is to visit Philippine Statistics Authority or PSA online website here. Once on the homepage, just click the tab of “CRS APPOINTMENT SYSTEM” in the left pane of the window similar below:

When directed to PSA Online Appointment Systems page, just click the “Civil Registration Service Appointment System” tab:

On the Privacy Policy page of PSA website, just scroll down after reading the Privacy Policy article then click the I Agree button.

When the Important Reminders prompt is displayed on the screen, just read then click the Next button:

On the page similar below, specify the region of PSA outlet you wish to set an appointment by clicking the drop down button of — Choose a Region — and selecting the region you prefer to visit.
Likewise, select also the municipality of the region where you wish to set your appointment by clicking the drop down button below the selected Region and selecting your preferred municipality. Then click Next button.
Step 6
Then on the similar page shown below, provide your personal details and contact information or of the person that is making the personal appointment to the preferred PSA outlet. Afterwards, tick the check box after agreeing to the Data Privacy Act statement then click Next button.
Step 7
After clicking the Next button, an OTP verification code will be sent to your registered email address. Just enter this code to the space provided below then click the Verify button.
Step 8
Select the type of certificate (Certificate of Live Birth) you wish to request and input your name or the name of the person who will be declared in the certificate or the owner of certificate. Then click Next button.
Step 9
Now choose the date you prefer for the appointment by clicking the number until the tile turned into dark blue.
Step 10
Then on the selected date, choose the time your prefer on you appointment that still has available slots. Then click Next button.
Step 11
You will then need to review the details of your appointment prior to continuing to next step. Just click Confirm button if all details are correct.
Step 12
Once you have verified the details of your appointment, it will now be successfully booked. Just click Next button to continue.
Step 13
Once booked, you will now get your appointment slip similar below. This slip contains the details of your appointment schedule you have selected during this entire procedure.
Step 14
Your appointment slip will be needed as proof of your appointment schedule thus you will be needing to print and bring it on your schedule date. Just scroll down until the bottom page then click the Print Appointment From to print your slip.
Step 15
Below is the sample of printed Appointment slip. This slip is a 2-page document wherein the second page contains the procedures for application.
Below Important Reminders and Procedures for Application were also printed and included in your slip. However, they were iterated here in cases you wish to learn more about the PSA Birth certificate appointment system prior to online application.
Important Reminders
- Be at the PSA outlet at least 30 minutes before your appointment schedule
- Bring valid IDs (document owner/s and authorized representative) that match to the names encoded.
- Copy issuance, authentication and annotation of Birth, Marriage and Death Certificates costs ₱155.00 per copy. while CENOMAR is ₱210.00
- Authorized representative is not allowed to delegate to another person his/her authority to request for civil registry documents.
- An authorized representative is only allowed to request for 2 unrelated individuals.
- Minors are not allowed to book an appointment.
- Securing an appointment is free of charge and CRS appointment slip is non-transferrable.
- For concerns and inquiries, transact with authorized PSA personnel only.
Procedures for Application
- Present the CRS appointment slip (printed or digital copy) to the information marshal for validation purposes.
- Pass through the Health and Safety Protocols and present the COVID-19 Contact Tracing Application or the accomplished Health Survey Form.
- Get Application For (AF) and Queue Ticket Number (QTN).
- Present/submit the corresponding fees, QTN, accomplished AF, valid IDs, Authorization Letter/Special Power of Attorney (SPA) and all supporting documents to the transacting window for screening and payment.
- Check the Official Receipt (OR) and count the change, if any.
- Proceed to the Releasing Area on the scheduled date and time of release.
- Present and submit the OR, valid IDs, Authorization Letter/SPA and all supporting documents to the Releasing Officer.
- Check the correctness and completeness of the received document.
Final Thought
This procedure can also be applied to other civil documents you wish to request from PSA like Marriage Certificate, Death Certificate and CENOMAR (Certificate of No Marriage). The procedure is the same across all these documents, except that you just have to select the certificate type you wish to request. Personally, I recommend that you get your own PSA Birth certificate this way instead of requesting it from any other agency or center which offers the same service for the following reasons:
- First, this is the cheapest way to get since you only pay the actual PSA processing/paper fee. (Of course third-party agency adds their own processing fee)
- Lastly, for your own security and privacy. This kind of documents hold your personal identity and you don’t want it to fall into the hands of any stranger and become a victim of identity theft. So it would be better if you personally claim it from the PSA outlet.
